VERTEX was retained to evaluate the cause and origin of reported water/glycol impact to mechanical equipment located in a school’s boiler room. The investigation included a site inspection, interviews with school maintenance staff, and a review of the boiler operational and maintenance logs to determine if proper corrective maintenance was being performed on the boiler system.

VERTEX’s investigation concluded that the 4-inch flanged Safe-Flex flexible coupling located on the discharge of variable speed secondary hot water pump P-4 failed due to a combination of wear and tear, misalignment, and excessive operational stress. VERTEX assisted the school with the development of a remediation cost estimate to repair and replace all equipment in the boiler room which had been impacted with water/glycol as well as evaluating the potential environmental impact of this failure.
